概念
- 通常は膵管と胆管は十二指腸壁内のOddi括約筋よりも上流で合流するもの。すなわち十二指腸の壁外で合流するものをいう。
- 膵液が胆管内に流入することで、胆嚢や胆管に慢性の炎症を起こしうる。
- 十二指腸乳頭部より膵胆管合流部までの距離が長くなると合流異常とみなされる。成人の場合10mm、小児の場合5mmで合流異常と考えられている。
- 総胆管拡張症の原因のひとつとなりえ、また胆嚢癌や胆管癌の危険因子の一つとなりうる。

病態
- 胆石を20%合併している。胆嚢炎や胆管炎症状をきたしてしまう。
