71歳女性．心窩部不快感を主訴に当科を初診した．腹部超音波検査にて肝S4に最大径35 mmの多房性嚢胞性腫瘤を認めた．肝粘液性嚢胞性腫瘍と術前診断し肝左葉切除術を施行した．病理組織学的には嚢胞内腔は異型の乏しい一層の単層円柱上皮で覆われており，その直下には卵巣様間質を認めた．嚢胞壁の一部に胆汁の沈着を認めたが，画像所見と同様に病理学的にも胆管と嚢胞内腔の交通は認めなかった．
