• 乳房にしこりとして感じられる。
• 腫瘍様病変：乳癌との鑑別が必要
• 乳房の間質が線維化　→　乳管の狭窄・閉塞　→　末梢で分泌物が貯留　→　乳管の拡張(乳管分泌物の濃縮したもの、脱落した細胞成分)　→　乳管の破綻　→　乳腺炎　→　硬結形成

疫学

• uncommon condition
• 40-50代で子供を産んだ女性でみられる。　→　may be a reaction to stagnant colostrum

病理

• 分泌物うっ滞、乳管拡張、乳管内円形拡張浸潤
• marked dilatation of large ducts with fibrosis and chronic inflammation (Q book p. 256)

• Usually the inflammatory changes are confined to an area drained by one or several of the major excretory ducts of the nipple. There is increased firmness of the tissue, and on cross-section dilated ropelike ducts are apparent from which thick, cheesy secretions can be extruded.
• 乳管には分泌物、剥離した細胞のほかに白血球、脂肪を蓄積したマクロファージが認められる。
• 乳管上皮は破壊されている。
• (本疾患に特徴的なのは)リンパ球や形質細胞が浸潤しており、時に周囲の間質に肉芽腫を認めることである。
