定義
- パルスオキシメトリによる酸素飽和度低下(SpO2)を伴う動脈圧酸素分圧低下(PaO2)をいう。小児の場合、room airでSpO2<94%とされている。(PALS AHAガイドライン2010年準拠 p.38)
原因
- 外呼吸:肺呼吸の障害(肺での換気やガス交換の障害)
- 内呼吸:組織呼吸の障害
低酸素血症の原因となりにくいもの
- 貧血:酸素を結合するヘモグロビンは減少しているものの、存在するヘモグロビンの酸素結合能は保たれており、酸素飽和度は正常である。酸素飽和度∝酸素分圧であり、低酸素血症になることはない。
- 低酸素血症:血液中の酸素飽和度低下
- 組織低酸素症:組織における酸素欠乏状態
- 両者は同一ではない。低酸素血症であっても、酸素運搬の上昇(ヘモグロビン濃度上昇)や血流量(心拍出量)の増大により組織での酸素需要を満たすことができる。一方、血液中の酸素飽和度が適正であっても、血流量の低下やヘモグロビン濃度の低下により組織での酸素需要を満たせないことがある。
