概念
- 生理的には、呼気時に単一音、吸気時に分裂音を聴取するが、これが呼吸によらず分裂音のみを聴取する場合を表現して言う。

固定性分裂が起こるメカニズム
- 生理的な状態では、吸気で胸腔内が陰圧となり、肺のキャパシタンスを上げて(血管抵抗を低下させて)P2がA2より遅れるようになる。ASDでは慢性的なvolume loadにより肺のキャパシタンスが上がりきった状態になっている。このために肺動脈弁を挟んだ圧較差は小さくなり、生理的な状態に比べP2はA2よりもさらに遅れる。(PHD.35)
- 心房中隔欠損の欠損孔からのシャント血流により、肺血管に容量負荷がかかりコンプライアンスが上昇。これによりP弁を挟んだ圧較差が低下する、ということか。
- 肺のキャパシタンスは上がりきっているので、(1)呼気による静脈還流量の増加であっても肺動脈弁を挟んだ圧較差は生理的な状態に比べて大きくなりにくい、また(2)ASDの欠損孔を介して呼気により増加した静脈血は左房とバランスを取ることができる(呼気時にleft-right shuntが減少)ので、P2とA2は呼吸による影響を受けない。(PHD.35)
