症例は72歳，女性．近医でアミラーゼ高値を指摘された．上部消化管内視鏡検査では十二指腸下行脚に3型の腫瘍を認め，生検で腺癌と診断された．CT，MRIでは粘液癌が疑われ，亜全胃温存膵頭十二指腸切除術を施行した．術中，腹膜播種を認めた．病理組織診断は，原発性十二指腸癌，粘液癌であり，副膵管内腫瘍栓をともなっていた．術後，化学療法を施行することにより2年8カ月生存した．
