- 症例は90歳男性．胸部苦悶で救急搬送され，腹部CTで門脈ガスと回腸浮腫を認めた．重症虚血性小腸炎と診断，循環不全のため保存的加療として救命し得た．その後腸閉塞となりイレウス管造影で回腸狭窄を認めたため，腹腔鏡補助下小腸切除術を施行し軽快した．急性腹症での門脈ガスは腸管壊死が示唆され緊急手術となるが，全身状態から手術困難な場合の保存的加療について，腸間膜血流維持を中心に今後検討していく必要がある．
