意義

生化学検査で知りうることは、血清タンパク、アミノ酸、窒素化合物、血清酵素、電解質、脂質など多岐にわたる。それぞれの項目には、反映する生体内の反応がある。生体内の反応の変化を知ることは生体の現在おかれている状態を理解する手助けとなる。生体内でおこっていることを基本的診察のみで100％知り得ることはできない。したがって生化学検査によって生体におこっている状況をとらえ、より確かな病態の把握をおこなうことが可能になる。

方法

患者から静脈血を採血し、血清用スピッツに注入し遠心により血清だけ分離し、血清を各項目に対応した試薬によって各検査項目に特異的な反応をおこし吸光度を求め定量する。

注意

生化学検査のみを行うことは少なく、同時に血算や凝固の検査もおこなうことが多いと考えられる。凝固や血算のスピッツには凝固反応を起こさせないように、EDTAやクエン酸Naなどがスピッツ内にあらかじめ入れてある。生化学検査のスピッツを血算や凝固のスピッツより後に採血すると、生化学検査のスピッツにEDTAやクエン酸が混入する可能性がある。もし混入すると陽イオンがキレートされ生化学検査の電解質の結果が狂ってくる。また、スピッツを力一杯混和すると、溶血をきたし赤血球内容物が溶出しLDH、AST、Kの値が増加する。
