脾梗塞を合併した伝染性単核球症の1例

症例は22歳男性．持続する高熱と右季肋部痛のため，当院へ救急搬送となった．伝染性単核球症と診断したが，腹部造影CT検査で脾梗塞を認め，入院加療とした．入院後，炎症反応は増悪し左側腹部痛が出現，脾梗塞も増悪したため，ステロイドパルス療法を施行した．これにより，自覚症状と炎症反応は改善した．脾梗塞を合併した伝染性単核球症の報告例は自験例を含めて9例とまれであるが，認知しておくべき合併症と考えられた．
