概念
疫学
- 成熟児の約3%に認められる。(QB.O-79)
- 新生児期では5%、1歳児には1%程度に見いだされる。2500g未満の低出生体重児では20-30%の発生頻度。(SURO.142改変)
病態
- 乳児期には組織障害は認められず、18ヶ月～2歳以降に精細管萎縮、間質増生がみとめられてくる。(SURO.142)
停留精巣によるリスク
- 鼡径ヘルニアの合併リスク 90%
- 性索軸捻転が起こりやすい
- 外傷を受けやすい
- 悪性腫瘍
- 精巣腫瘍の10%は停留精巣から発生。停留精巣は正常の精巣に比べ40倍の確率で悪性化する。
治療
- 経過観察：1-2歳で自然降下することが多い。
- 外科手術：1-2歳を過ぎても(SPE.618)、あるいは12ヶ月まで(QB.O-79)に精巣が降下しない場合には陰嚢内精巣固定術を行う。
