Peripheral blood stem cell transplantation (PBSCT), also called peripheral stem cell support, is a method of replacing blood-forming stem cells destroyed, for example, by cancer treatment. PBSCT is now a much more common procedure than its bone marrow harvest equivalent, this is in-part due to the ease and less invasive nature of the procedure. Studies suggest, that PBSCT has a better outcomes in terms of the number of hematopoietic stem cell (CD34+ cells) yield.

Immature blood cells hematopoietic stem cells in the circulating blood that are similar to those in the bone marrow are collected by apheresis from a potential donor (PBSC collection). The product is then administered intravenously to the patient after treatment. The administered hematopoietic stem cells then migrate to the recipient's bone marrow, a process known as stem cell homing, where the transplanted cells override the previous bone marrow. This allows the bone marrow to recover, proliferate and continue producing healthy blood cells.

The transplantation may be autologous (an individual's own blood cells saved earlier), allogeneic (blood cells donated by someone else with matching HLA), or syngeneic (blood cells donated by an identical twin). The procedure typically lasts for 4–6 hours, depending on the donors total blood volume.

Preparation prior to PBSC Collection

GCSF

Granulocyte colony stimulating factor (GCSF) are naturally occurring glycoproteins that stimulate white blood cell proliferation. Filgrastim is a synthetic form of GCSF produced in E.coli. PBSC donors are given a course of GCSF prior to PBSC collection, this ensures a better outcome, as stem cell proliferation increases, thus increasing the number of peripheral stem cells in circulation. The course is usually given over a 4 day period prior to PBSC collection. Mild bone pain usually results due to the excessive stem cell crowding within the bone marrow.

Complications

Since allogeneic PBSCT involves transformation of blood between different individuals, this naturally carries more complications than autologous PBSCT. For example, calculations must be made to ensure consistency in the amount of total blood volume between the donor and recipient. If the total blood volume of the donor is less than that of the recipient (such as when a child is donating to an adult), multiple PBSCT sessions may be required for adequate collection. Performing such a collection in a single setting could result in risks such as hypovolemia, which could lead to cardiac arrest, thus health care providers must exercise careful precaution when considering donor-recipient matching in allogeneic PBSCT

Vascular injection of sub-lethal doses of QDs in B. mori larvae caused time- and dose-dependent damage in the hematopoietic organ and hematocytes. QDs with the maximum emission wavelength of 530 nm (QDs530) were quickly observed in cystocytes and plasmacytes, and gradually bleached their green fluorescence, followed by a decrease in peripheral hematocytes. Additionally, the proportion of abnormal hematocytes increased. In marked contrast, QDs with the maximum emission wavelength of 720 nm (QDs720) were quickly surrounded by hematocytes and subsequently enriched in cystocytes like the human's leukocytes, but with weaker cytotoxicity. QDs exposure promoted the mitotic nucleus in prohemocytes and hematocytes similar to peripheral blood stem cells in humans, but aggravated apoptosis. A decrease in hematopoiesis was accompanied by shrinkage and death of hematopoietic organs via an increase in reactive oxygen species. QDs with smaller size resulted in more severe hematopoiesis toxicity.

Adult human peripheral blood mononuclear cells (hPBMCs) may be an ideal cell source for such therapies. This study examined the survival and migration of pre-induced hPBMCs three months after subretinal transplantation in the retinal degeneration slow (rds) mouse model of RP. Materials and Methods: Freshly isolated adult hPBMCs were pre-induced by co-culture with neonatal Sprague-Dawley (SD) rat retinal tissue for 4 days in neural stem cell medium. Pre-induced cells were labeled with CMDiI for tracing and injected into the right subretinal space of rds mice by the trans-scleral approach. After two and three months, right eyes were harvested and transplanted cell survival and migration examined in frozen sections and wholemount retinas. Immunofluorescence in whole-mount retinas was used to detect the expression of human neuronal and photoreceptors protein markers by transplanted cells. Results: Pre-induced adult hPBMCs could survive in vivo and migrate to various parts of the retina. After two and three months, transplanted cells were observed in the ciliary body, retinal outer nuclear layer, inner nuclear layer, ganglion cell layer, optic papilla, and within the optic nerve. The neuronal and photoreceptor markers CD90/Thy1, MAP-2, nestin, and rhodopsin were expressed by subpopulations of CM-DiI-positive cells three months after subretinal transplantation. Conclusion: Pre-induced adult hPBMCs survived for at least three months after subretinal transplantation, migrated throughout the retina, and expressed human protein markers. These results suggest that hPBMCs could be used for cell replacement therapy to treat retinal degenerative diseases.
