症例は79歳，女性．胃潰瘍治療中，右季肋部痛が出現し入院．上部消化管内視鏡検査にて十二指腸下行部に魚骨と考えられる異物を認め，腹部CT検査で膵頭部やや背側への脱出を認めた．限局性炎症と考え保存的治療を行う方針とし内視鏡下に36mm長の魚骨を摘出した．術後厳重に経過観察したところ症状の増悪なく軽快した．比較的稀な魚骨の十二指腸穿通に対し内視鏡的除去後，保存的に治癒し得た症例を経験した．
