- 急性壊死性食道炎の4例を経験した.4例とも初発症状は吐血で,上部消化管内視鏡検査では特徴的な黒色食道を呈した.発症時の基礎疾患は,3例がケトアシドーシス,2例が糖尿病であった.3例は保存的に軽快し,死亡例を1例認めたが死因は急性壊死性食道炎によるものではなく,基礎疾患である敗血症が予後を規定した.急性壊死性食道炎はまれな疾患ではあるが,緊急内視鏡における鑑別診断として念頭に置くべきであると考える.
