Onychogryphosis (also known as "Ram's horn nails"[1]) is a hypertrophy that may produce nails resembling claws or a ram's horn, possibly caused by trauma or peripheral vascular disorders, but most often secondary to neglect and failure to cut the nails for extended periods of time.[2]:783-4[3] Onychogryphosis is most commonly seen in the elderly, and some recommend avulsion of the nail plate with surgical destruction of the matrix with phenol or the CO2 laser, if the blood supply is good.[2]:783-4[4]:659
Severe congenital Onychogryphosis affecting all twenty nailbeds has been recorded in two families, who exhibit dominant inheritance of the allele. [5][6] Congenital Onychogryphosis of the fifth toe (the little toe) is fairly common, but asymptomatic and seldom brought to the attention of medical professionals. Rather, it is brought to the attention of manicurists who routinely file the clawed toenail flat.

- 87歳, 男性。数年前から外傷の既往なく左第1趾爪が変形しだし, 約1年前から痛みだしたが, 誰にも見せず放置していた。初診時, 左第1趾爪は白濁肥厚, 迂曲変形し後爪郭に陥入して反応性肉芽と潰瘍を形成していた。抜爪により自他覚症状は改善した。直接鏡検で白癬菌を認めたため, 爪白癬に伴う爪甲鉤弯症と診断した。特異な臨床像を呈した爪甲鉤弯症と考え報告した。
