関: グレーブス病

概念

眼球突出、眼球運動障害、眼瞼浮腫、結膜充血、視神経障害などの眼病変がバセドウ病患者にみられることをいう。

病態生理

眼窩内組織(外眼筋、球後結合組織)にTSH受容体を介する自己免疫機序を開始、外眼筋肥厚、脂肪組織増生、涙腺肥大を生じ、結果として眼窩内圧が上昇する。これにより眼球突出をきたす。

症状

徴候

ステルワーグ徴候 Stellwag sign：瞬目の減少
グレーフェ徴候 Graefe sign lid lag：下方視に際して上眼瞼が下降しないため強膜上部が露出　←　上眼瞼筋の緊張
ダルリンプル徴候 Dalrymple sign：眼瞼開大。交感神経優位となりMuller筋が異常に収縮することによる
メビウス徴候 Mobius sign：輻輳不全
