舌咽神経疾患、舌咽神経

• 一側舌咽神経支配領域に発作性疼痛を来す疾患
• 女性に比べて男性に多く、30-40歳代に好発する。
• 三叉神経痛の1/10～数十分の一ないし1/100の頻度とされる。
• 咳嗽、くしゃみ、嚥下、発声なとにより誘発される
• 激しい一過性の鋭い痛みが耳、舌後方、扁桃窩、顎の下方に起こる。
• 一側の耳が赤くなることがある。
• 鑑別疾患：三叉神経痛(第3枝)、扁桃炎、舌咽咽頭炎、歯疾患
• 治療：本態性の場合にはカルバマゼピン・フェニトインなどの抗痙攣薬内服、無効の場合は舌咽神経ブロック、神経血管減圧術。
