  • 何らかの心機能の障害により心拍出量が低下し、臓器還流不全を来した(ショック)病態。



  • 駆出不全型、駆出障害型 :急性心筋梗塞、重症心筋炎、急性弁不全
  • 充満不全型、心室充満障害:急性心タンポナーデ、緊張性気胸


  • 左心不全性心原性ショック
  • 重症不整脈性心原性ショック
  • 右心負荷性心原性ショック


  • 1. 低血圧:収縮期圧90mmHg以下。高血圧患者では発症前値より30mmHg以上の低下。
  • 2. 尿量の減少:時間尿20mL以下
  • 3. 不穏、意識レベルの低下
  • 4. 末梢循環不全に基づく皮膚の蒼白化、チアノーゼ、冷汗など ← 交感神経の緊張(末梢血管収縮)
→ 代謝性アシドーシス
  • 5. 肺うっ血:左室拡張末期圧の上昇により肺静脈圧が上昇する  ←  出血性ショックと違う点
  • 6. (心拍出量の低下の代償として)頻脈(>90回/分)
