ルイス式血液型

概念

• 各種の消化器癌や膵癌、胆肝癌の患者に高頻度かつ高濃度に検出される抗原である。
• ヒト大腸癌細胞株SW1116に対して作成された単クローン抗体の認識する糖鎖抗原であり、腫瘍マーカー検査に用いられる。
• 抗原は Gal1-3GlcNAc の繰り返し構造(I型糖鎖)を基本骨格とするルイスA(Lea)の血液型糖鎖にシアル酸が結合したシアリルルイスAである

基準値

• 0-37 U/ml (HIM.A-4)

分布

• 膵管、胆嚢、胆管、胃、気管支、唾液腺、前立腺、結腸、直腸などの上皮細胞上に分布

• 特に膵管、胆嚢、胆管に多い。

意義

• 膵管、胆嚢、胆管の腫瘍で高値

婦人科

• CA19-9：卵巣癌、子宮体癌、成熟嚢胞性奇形腫

• (腫瘍)細胞表面の糖蛋白に結合している糖鎖

• 癌の血行性転移を促進するらしい
• 正常の胆管膵管上皮、唾液腺でも発現

• 早期癌での陽性率は低い。

• 補助診断、治療再発のモニター

注意

• 血液型Lea陰性者(日本人の5-10%)では陰性を示す
