概念
- 成長期における長管骨骨幹端部の血行系の解剖学的特異性に起因する血行性感染
疫学
- 小児の疾患
- 女児より男児に多い
- 長管骨骨幹端部に発生しやすい(大腿骨＞脛骨＞上腕骨)
検査
- 発症時のX線像は正常。早期発見には有用ではない
- 1-2週間後に骨萎縮、骨膜反応出現
- 早期診断にはMRIが有用(T1 low, T2 high)
起炎菌
治療
- 薬物療法：抗菌薬静脈内投与。臨床症状が改善し、CRP、赤沈などの検査上も正常化しても2週間は抗菌薬の静脈内投与を継続する。その後、6-8週間は経口抗菌薬を服用する。
- 外科療法：骨膜下膿瘍の排膿と骨の開窓
