疫学

中年以降の女性に多い

誘因

ストレス、うつむき姿勢、暗所、抗コリン薬摂取

病因

隅角閉塞に伴って突然の眼圧上昇が起こる。

病態

SOP.94

急激な眼圧上昇　→　角膜浮腫混濁、結膜充血、毛様体充血

症候

SOP.94

浅い前眼房、中等度瞳孔散大、対光反射消失、視神経乳頭充血、浮腫
高眼圧(40-80mmHg)
(治療が遅れると)虹彩の実質の萎縮・脱色、瞳孔括約筋麻痺による瞳孔散大、水晶体前嚢下混濁、視神経萎縮

充血、角膜混濁、対光反射減弱、患側眼で散瞳傾向、視力低下

症状

眼痛、頭痛、ときに悪心・嘔吐
視野障害？
急激な眼痛、視力低下、霧視、虹視、充血、悪心・嘔吐、頭痛、徐脈 (SOP.94)

治療法

マンニトール(高張浸透圧薬)で硝子体容積減少、ピロカルピン(副交感神経刺激薬)で縮瞳させシュレム管からの房水流出を促進し、さらにアセタゾラミド(炭酸脱水酵素阻害薬)で房水産生を抑制する。その後、レーザー虹彩切開術、周辺虹彩切除術で虹彩に穴をあけ房水の流路を形成する(CBT QB vol2 p.491 SOP.104-105)
