内頚動脈閉塞症に対して急性期STA-MCA吻合術後に内頚動脈血栓内膜剥離術を施行した１例

症例は66歳女性で右片麻痺、失語で発症した。MRI で左島皮質に脳梗塞を認め、MRA で左内頚動脈閉塞を認めたので緊急STA-MCA 吻合術を行った。術後脳血管造影で左内頚動脈の閉塞は開存したが、高度狭窄が残存した。慢性期に内頚動脈血栓内膜剥離術を行い、リハビリテーションを行ったところ、歩行可能となり良好な経過をえた。
